Alex Mostoller Named 2025 Long Term Care/Rehab Practitioner of the Year!
At Ohman Family Living, excellence means giving our very best—not perfection. Today, we’re proud to celebrate Alex Mostoller, Director of Respiratory Therapy & Clinical Liaison, who has been named the 2025 Long Term Care/Rehab Practitioner of the Year by the Ohio Society for Respiratory Care (OSRC). This prestigious award, presented at the OSRC State Meeting on August 5, honors Alex’s leadership, initiative, and professionalism in reshaping respiratory care for complex patients.
Driving Real Change
Alex joined Ohman Family Living in 2016 and founded our high-acuity ventilator unit—a first-of-its-kind program in a skilled nursing setting. With a Bachelor of Science in Respiratory Care from Youngstown State University and certifications as a Registered Respiratory Therapist (RRT) and Registered Polysomnographic Technologist (RPSGT), Alex has built a career defined by innovation and impact.
His achievements include reducing hospital readmissions, expanding care for higher-acuity patients, and leading outreach efforts to strengthen partnerships with hospitals like the Cleveland Clinic.
One standout example from his nomination: Alex led the seamless admission of a critically ill patient with an LVAD, tracheostomy, ventilator dependency, and multiple comorbidities. His leadership in developing protocols, training staff, and coordinating across systems resulted in zero 30-day readmissions and a significantly improved quality of life. As Joshua Wallace, VP of Growth Strategies, shared:
“Alex is not only a highly skilled respiratory therapist—he is a leader who is actively reshaping how post-acute care is delivered.”
Compassion + Competence = Success
What sets Alex apart is his ability to pair clinical expertise with genuine compassion. He builds relationships, fosters teamwork, and motivates others through shared purpose. His influence reaches beyond our walls—presenting at the 2025 OHCA Winter Conference and serving on the Respiratory Care Advisory Board at Ursuline College.
He was motivated to lead his team and collaborate with the broader clinical staff to create a 5-star rated quality program, as determined by the Centers for Medicare & Medicaid Services (CMS). Since we began tracking readmissions in 2018—when Briar had a roughly 40% readmission rate—Alex’s leadership has helped reduce our risk-adjusted readmission rate to just 12.9%, even among some of the sickest and most frail patients we serve.
Alex has also reinvested in the respiratory therapy community by partnering with local colleges to provide real-world clinical experience. As a former educator at Youngstown State University, he remains deeply committed to building the next generation of clinicians, ensuring that excellence in care continues well into the future.
Reinforcing Our Commitment to Respiratory Excellence
This award is not just a win for Alex—it’s a reflection of our entire team’s dedication to setting and achieving the highest standards in respiratory care. By challenging the status quo and delivering evidence-based outcomes, we’re redesigning healthcare for aging adults and those with chronic respiratory conditions.
Alex’s recognition by OSRC is proof that we’re making a real difference—one patient at a time.
