In our daily operations we are often addressing key aspects of high-acuity respiratory care, particularly focusing on ventilator patient discharge planning and post-acute ventilator management. At our facility, we ensure that hospital discharge to Skilled Nursing Facilities (SNFs) is handled with utmost precision, particularly for those requiring skilled nursing for tracheostomy patients. Our dedicated team includes respiratory therapists available seven days a week, with staffing ratios carefully adjusted to meet the needs of complex care transitions. Our respiratory therapy team is supported by expert nurses who bring acute care experience, ensuring seamless management from hospital to SNF, and are trained to anticipate changes in patient conditions before they become critical.
1. What are your Respiratory Therapist (RT) staffing ratios for ventilator patients?
At Ohman Family Living, we prioritize patient safety and outcomes through acuity-based RT staffing. Our ventilator units are supported by:
- 24/7 on-site Respiratory Therapists
- 24/7 on-call Director of Respiratory Services
- Nurses with acute care experience, trained in ventilator management and early detection of clinical changes
This ensures rapid response and proactive care for ventilator-dependent patients.
2. Who manages your high-acuity respiratory patients?
Our interdisciplinary team is led by experienced physicians:
- Dr. Lawrence Goldstein, Attending Pulmonologist – 30+ years in acute care
- Dr. Matthew Pawlicki, Medical Director, – 20+ year medical practice and certified medical director
- Dr. Yue Yuan, Attending Hospitalist – 10+ years in acute care
They collaborate closely with our Director of Respiratory Therapy and Director of Nursing to deliver coordinated, high-quality care. Additional specialists are available as needed.
3. What types of complex respiratory patients can you accept?
We specialize in managing high-acuity, medically complex patients, including:
- Ventilator-dependent (weaning and non-weaning)
- Tracheostomy care
- Airvo therapy (up to 50 LPM)
- Corpak feeding tubes
- LifeVest monitoring
- Complex wound care – Certified Wound Specialist – Physician (CWS-P) onsite.
Our integrated care model helps reduce hospital readmissions and supports successful transitions from acute care.
4. How can I address continued COPD readmissions?
If COPD flare-ups keep landing you or your loved one in the hospital despite trying inhalers or other treatments at home, Non-Invasive Ventilation (NIV) can be a game-changer. NIV uses a comfortable mask to deliver air, helping you breathe easier without invasive tubes. It stabilizes your breathing, reduces shortness of breath, and helps prevent flare-ups that lead to hospital stays. A NIV combined with rehab exercises, oxygen therapy, medications, and diet guidance to keep you stronger and out of the hospital. At Briar, our goal is to break the cycle of hospitalizations.
5. How can we evaluate the quality of your care?
